My hospital tests came back normal, but… The primary cause of a burning sensation in the mouth is stress.

Hello. I’m Dr. Hwang Sang-cheol, and I specialize in researching and treating the autonomic nervous system. In my practice treating patients with burning mouth syndrome, I’ve found that stress is one of the most common underlying causes. It’s not uncommon for patients to experience persistent symptoms—such as a burning sensation on the tongue or a stinging, bitter taste in the mouth—even though medical tests show no specific abnormalities.

Stress affects our bodies in various ways.

Stress isn’t just caused by having a heavy workload. When various situations—such as financial burdens, failed investments, family conflicts, problems with a spouse or partner, or workplace relationships—persist, the body remains in a state of tension. The longer this tension lasts, the more the balance of the autonomic nervous system begins to be disrupted.

When the autonomic nervous system is disrupted, the body sends out various signals

When stress persists, the sympathetic nervous system becomes more active, making it difficult to fall asleep and often leading to stiffness in the neck and shoulders, as well as recurring headaches and indigestion. These changes may not simply be a sign of fatigue, but rather an indication that the body is in a state of constant tension.

Why does my tongue feel like it’s on fire?

Sensations in the tongue and mouth are controlled by various cranial nerves. When the autonomic nervous system becomes unbalanced and the nerves become hypersensitive, abnormal sensations such as a burning or tingling sensation may occur even in the absence of actual inflammation. As a result, it is not uncommon for various tests to reveal no specific abnormalities.

Even if the test results are normal, the symptoms are clearly present.

Many patients with burning mouth syndrome continue to report discomfort even after being told, “Your test results are normal.” This is because, while they may not be experiencing pain, functional changes are sometimes not easily detected by tests. Therefore, rather than making a definitive judgment about symptoms based solely on test results, it is important to take a comprehensive approach to assessing the patient’s condition.

Stress management is also an important management factor.

Not all cases of burning mouth syndrome are caused by stress. However, stress can be a significant factor in triggering or worsening symptoms. Getting enough sleep, maintaining a regular routine, and practicing stress-reducing habits can help manage symptoms; if symptoms persist, it is advisable to seek a proper evaluation.
